Product Eligibility and Clinical Approval Process

FIXX Regenerative Patches are dispensed only after a licensed FIXX provider confirms they are appropriate for you. This policy explains how that review works and what to expect.
1. Reservation Is Not a Purchase
Submitting a reservation through our online form, or paying a deposit if applicable, holds your place in line and schedules your clinical review. It is not a guarantee that any product will be approved, shipped, or dispensed to you.
2. Who Is Eligible
To be considered for FIXX Regenerative Patches, you generally must:
- Be at least 18 years old
- Reside in [Florida — peptide availability and telehealth regulations vary by state]
- Not be pregnant, trying to become pregnant, or breastfeeding
- Have no listed contraindicated conditions — see our Medical Disclaimer
- Be able to attend a verification visit (in person or via telehealth, as offered) with a licensed FIXX provider
Meeting these general criteria does not guarantee approval. Final eligibility is determined individually by your provider.
3. The Verification Process
- Reserve your spot. Complete our online reservation form with your basic information and health history.
- Schedule your verification visit. You'll book a visit with a licensed FIXX provider through our scheduling platform at fixx.janeapp.com.
- Clinical review. During the visit, your provider will review your health history, current medications, and goals to determine whether a FIXX Regenerative Patch is appropriate for you, and if so, which formulation.
- Approval or denial. You will be notified of the outcome of your review. If approved, your order will be processed and shipped or made available for pickup. If not approved, see Section 5 below.
4. Provider Discretion
FIXX providers exercise independent clinical judgment. A provider may decline to approve a product for any medically appropriate reason, may request additional information, may recommend a different formulation than the one originally reserved, or may refer you to your primary care physician. FIXX reserves the right to decline service to anyone for any lawful reason, including but not limited to failure to meet eligibility criteria, safety concerns, or incomplete information.
5. If You Are Not Approved
If your provider does not approve a product, then your credit card on file will not be charged for a product that has not been approved and dispensed.
6. Re-Verification
Approval is valid for 12 months. Continued or repeat use requires a follow-up verification visit before additional product is dispensed.
7. Accuracy of Information
You agree to provide complete and accurate information about your health history and current medications during the reservation and verification process. Providing false or incomplete information may result in denial of service and may affect your safety.
